New & Returning Applicants must complete all of the following forms in order to be considered for employment. Please retain copies of all paperwork for your records.
The Staff application is an online application. The application takes between 10-15 minutes to complete. Please be sure to review it, ensure you have all the information it requests handy, and set aside the correct amount of time to be able to complete it entirely. Partially completed applications will not be accepted.
Each applicant needs to submit two reference forms. The reference form is an online form. Click on the button below and send your person of reference the link to complete the form and submit on your behalf.
Download and complete the W-4 form form and submit via email or mail to the Badger office.
Download and complete the I-9 Form and submit to the Badger office. Your I-9 must be accompanied by original IDs – please note the acceptable forms of ID.
Staff Health Form
The Staff health form is an online form. The form takes between 10-20 minutes to complete dependent on the answers to some questions that may request a more detailed response. Please be sure to review it before starting to complete the form to ensure you have all the information it requests handy, and then set aside the correct amount of time to be able to complete it entirely. Partially completed applications will not be accepted.
All staff members must read the Badger Employee Handbook, sign and submit.

Important: Working Papers are required if you will be under the age of 18 at any point during your employment. (These are usually obtained at your school or your town hall, depending on where you live).
