New & Returning Applicants must complete all of the following forms in order to be considered for employment. Please retain copies of all paperwork for your records.

Staff Application

New to Badger?

Applications must be entirely filled out to be considered, partially completed applications will not be accepted. Once your application is submitted, we will reach out to schedule an interview.

Staff Dashboard

If you are a returning staff member or have already completed an application, you can access the staff dashboard using your designated username and password.

Reference Form

Each applicant needs to submit two reference forms. The reference form is the pdf below. Click on the button below and send the pdf to your person of reference. Reference forms can be submitted via email or mail.

W-4 Form

Download and complete the W-4 form form and submit via email or mail to the Badger office.

I-9 Form

Download and complete the I-9 Form and submit to the Badger office. Your I-9 must be accompanied by original IDs – please note the acceptable forms of ID.

Staff Health Form

Please download and complete the staff health form. Please be sure to review it before starting to complete the form to ensure you have all the information it requests handy, and then set aside the correct amount of time to be able to complete it entirely. Partially completed applications will not be accepted.

Employee Handbook

All staff members must read the Badger Employee Handbook, sign and submit.
The digital signature field is located at the end of the handbook.

Working Papers

Important: Working Papers are required if you will be under the age of 18 at any point during your employment. (These are usually obtained at your school or your town hall, depending on where you live).

Read and sign the Instructor Guide. This guide applies to Instructors only.

Heads Up Course

By taking this free, online course and using what you learn, you will be well positioned to improve the culture of concussion. Your actions can help create a safe environment for young athletes so that they can stay healthy, active, and thrive – both on and off the playing field.

All Eyes On You

All staff are required to complete an 8-minute safety training video and exam. You may watch the video and take the exam up to 3 times until you pass.
